Refer a patient
For emergency departments, hospitals, jails and corrections, probation and drug courts, primary care, and community organizations. We take warm handoffs and can often see your patient the same or next business day.
How referrals work
- Send the referral using the form here, by fax, or by phone. A signed patient consent lets us update you on enrollment.
- We call the patient the same business day and book a video visit, often within 24 hours.
- First visit: individual counseling, then a physician or nurse practitioner who sends the buprenorphine/naloxone prescription to the patient's pharmacy before the visit ends.
- We close the loop. With consent, we confirm enrollment and can share attendance and screen documentation for courts or probation.
Good fits
- ED patients treated for overdose or withdrawal, including those started on buprenorphine in the ED who need a follow-up prescriber
- People leaving jail or prison, especially in the first two weeks after release
- Probation, parole, and drug-court participants who need documented treatment
- Primary care patients you'd rather not manage buprenorphine for
- Pregnant patients (we coordinate with OB)
Not a fit right now
Patients who need medically supervised withdrawal from alcohol or benzodiazepines, or who need inpatient or residential care. We'll help find the right level of care and take them when they step down.
